Appointment Policy
To allow ample time for all patients and scheduled surgical procedures, we operate primarily by appointment. We appreciate your understanding that emergencies will receive priority and may cause an appointment delay. Please realize that we make a sincere attempt to see each client on time.
For your convenience, drop-off appointments are available. A 'drop off' means you could bring your pet at a more convenient time, usually in the morning, and leave him/her with us for our doctors to examine in between appointments or at a time purposely reserved for admitted patients. Once the doctor is done, you will receive a call to go over the diagnosis and to give you discharge instructions.
For the safety of our staff and all the animals in our care, we require that all vaccinations be up-to-date for any hospitalized patients. Dogs are required to be current on rabies and distemper. Cats are required to be leukemia negative and current on rabies, distemper and leukemia vaccines.
Patient Arrival Policy
Even though we make every effort to make our patients feel comfortable during visits, they may be a little uneasy about new people, new surroundings and other pets. This is one of the reasons we ask you to restrain your pet. For your protection, and that of others, all dogs must be on a leash and properly controlled while in the reception area. All cats must be presented in an appropriate cat carrier or on a leash.
Financial Policy
All fees are due at time of services rendered or patient discharge. For your convenience, we accept credit cards, debit cards, cash, and personal checks. In addition, we offer CareCredit through GE Financial Services as a monthly billing option. Applications, which can be processed in minutes, are available on line and at our front desk. There are a variety of pet insurance programs also available for you to consider.
Privacy Policy
We do not collect or give out any information to any third party vendors or internet businesses. No information will ever be disclosed to outside parties from us or our website hosting company at www.evetsites.com. It's that simple! And we strictly adhere to this policy. University Park Veterinary Hospital recognizes the importance of protecting the personal privacy of its clients, its patients and those individuals who simply visit our site.
Your privacy is also protected in our hospital. At no time is any information given out about the client or the pet without the consent of the owner.
Medical Records Policy
The state of Indiana considers a pet's medical history to be private information. Therefore, we are required to have written authorization to share any of your pet's information on your behalf. Boarding kennels, groomers, or other hospitals may all require information such as vaccine status, health status or results of blood tests in order for your pet to be at their facility. We will keep a copy of this written authorization in your file if you give us your permission to share this information about your pet.
